Reads every declared signal and records what it finds, as the standing reflection routine does after scan and process.

- The same sources as list; the evidence last written per signal is kept beside the declarations.
Output and state effect#
- A signal past its level with no open class defect of its product and shape records one: class, shaped, its declared title, impact and mechanism as the repair plan, the crossing subjects as evidence, ranked with its declared priority, and carrying `oko reflection signals remaining` as its completion probe. With an open class defect of that product and shape, the crossing is a note on it. The same crossing read again adds nothing (`covered, evidence unchanged`). Oko Desktop's Record crossings button runs it.
Refusals and safe exits#
- As list. A defect the register refuses to record (an open defect of the same title) refuses the pass with that cause.
